VS Code sometimes doesn't understand combos

How comes VS Code sometimes does and sometimes doesn't turn \< into ⟨ ? Am I hitting some kind of combo -breaker key?

It isn't. If you aren't aware of it, then it is likely not installed.

I have no idea what could be causing the abbreviation mechanism to fail for you then.
